Complete the application form to get started becoming a volunteer at Faith Regional.

Complete the Applicant Disclosure and Authorization Form.

Complete the Confidentiality Agreement and Continuing Education Quiz here. You will need have read the Volunteer Manual below in order to complete the education quiz.

Complete the Authorization for Release of Information for marketing and media purposes.

When you come for your interview with Volunteer Services, an Adult and Child Abuse/Neglect check through the State of Nebraska Department of Health and Human Services will be conducted. You will be asked to log in to the registry portal using a hospital computer. You will be asked to provide the information below. The document will need to be notarized and submitted for verification. Notary Public personnel are available at the hospital upon request.

• First, middle and last name
• Date of birth
• Social Security number
• Current address
• Current phone number
• All other names, including maiden, former married name, or nickname
• Names and birthdates of any children who have lived with you, including any siblings
• All previous addresses you have resided at

Complete this Vaccination Form. If you are under the age of 19, you also need to complete the Vaccination Parental Consent Form.
