When you're ready to become a volunteer at Faith Regional, there are a few things we need you to complete, see below. If you have any questions, call us at (402) 644-7529. You'll also have a short interview scheduled with a member of the Volunteer Services staff.

Background Check
Confidentiality Agreement/Continuing Education Quiz
Complete the Confidentiality Agreement and Continuing Education Quiz here. You will need have read the Volunteer Manual below in order to complete the education quiz.
Release of Information Authorization
Complete the Authorization for Release of Information for marketing and media purposes.
State of Nebraska Adult and Child Abuse/Neglect check
When you come for your interview with Volunteer Services, an Adult and Child Abuse/Neglect check through the State of Nebraska Department of Health and Human Services will be conducted. You will be asked to log in to the registry portal using a hospital computer. You will be asked to provide the information below. The document will need to be notarized and submitted for verification. Notary Public personnel are available at the hospital upon request.
- First, middle and last name
- Date of birth
- Social Security number
- Current address
- Current phone number
- All other names, including maiden, former married name, or nickname
- Names and birthdates of any children who have lived with you, including any siblings
- All previous addresses you have resided at
Vaccination Form
Complete this Vaccination Form. If you are under the age of 19, you also need to complete the Vaccination Parental Consent Form.
Volunteer Manual
You will need to read the Volunteer Manual before your scheduled interview. If you have any questions, call us at (402) 644-7259.
Parental Consent
If you are under the age of 19, parental consent is required. Your parent or legal guardian is required to appear in person, with a state-issued identification card in order to complete the background check.
