Faith Regional Health Services generally accepts any type of insurance provided for most cases. If you have a question regarding your insurance coverage please contact your insurance company directly by calling the number listed on the back of your insurance card. Your insurance company will be able to tell you if they cover a specific treatment or procedure or if independent providers are in your network.


Common Insurance Terms and Definitions

Deductible/Co-Pays/Co-Insurance Payments

Many insurance companies have amounts that as a patient, you must pay. These are called deductible, co-pay, or co-insurance payments. You will be asked to pay your deductible, co-pay, or estimated co-insurance payment at the time of service. Any amounts not paid at the time of service will be billed to you. You will also be billed for any other amounts not covered by your insurance. If you have a question about why your insurance company did not pay part of a claim, please call your health insurance company directly.

Explanation of Benefits (EOB) or Explanation of Payment (EOP)

An explanation of benefits (EOB) or an explanation of payment (EOP) are documents mailed to you by your insurance company with a detailed listing of how your insurance company processed your claim or bill.

Managed Care Health Insurance Plans

Managed Care health insurance pays for your healthcare needs and provides your healthcare within the plan's selected network of providers. Most Managed Care health plans require members to choose a Primary Care Physician (PCP) and obtain referrals from their PCP to see a specialist.

What is a Primary Care Physician (PCP)? A Primary Care Physician (PCP) is a provider who will provide and coordinate all of your medical care. PCPs have specialties in:

  • General Internal Medicine
  • Family Practice/General Practice
  • Pediatrics

What is a Referral? A referral is permission from your physician to seek care. Things to remember about a referral:

  • You need to confirm the referral is for a service covered by your health insurance plan.
  • The referral should be to a provider within your health insurance plan network.
  • You may need to check for limitations on the referral such as the number of visits allowed or expiration date.
  • Contact your PCP about a referral BEFORE your appointment.

Precertification/Prior Authorization/Referrals

Many health insurance plans have precertification or prior authorization requirements you must meet for specific healthcare services. Information about these requirements is usually listed on the back of your healthcare insurance card. You can refer to your card, your benefits handbook, or contact customer service for your healthcare insurance plan to inquire about your responsibility to complete these requirements.

Please verify your health insurance coverage and hospitalization requirements and complete any necessary precertification documents before your admission, making the admission and billing processes easier. After you are discharged, the appropriate medical claims will be submitted to your insurance company. If your group insurance requires an employer claim form, please complete yours and submit it upon discharge. As a courtesy to you, we will process your insurance claim.


What if I Don't Have Insurance?

Faith Regional Health Services provides medically-necessary services to patients without regard to their ability to pay. If you do not have insurance, you may be eligible for financial assistance.
