One Chart l Patient is a secure, online web portal that offers personalized access to a part of your medical record. There is no cost for signing up for One Chart l Patient.

Is One Chart l Patient a secure site?

Yes. Your privacy is important to us. This web site is designed to meet federal laws involving the privacy and security of your personal information. All information sent is encrypted to ensure your privacy is maintained. Additionally, the information shared is protected by layers of security within our infrastructure to further safeguard your information.

What features are available?

One Chart l Patient is a convenient way to:
  • Ask your health care team questions (response time is within 24 business hours)
  • Schedule appointments
  • Pay your bill
  • Review your test results, health information and instructions from your last visit. Results are released at 1:00 p.m. for all categories listed below:
    1 Day: Normal Results are released.
    2 Days: Abnormal Results are released.
    5 Days: Outpatient Imaging Results are released.
    14 Days: Pathology Results are released.
    30 Days: Sensitive Results are released.
  • Request updates to your list of medications, health problems, or allergies.
  • Request prescription renewals

Is there an app available for my mobile phone or tablet?

Yes. Search and download the MyChart in the App Store (Apple) or Play Store (Android). Once downloaded, open My Chart and select Nebraska Medicine as your provider. If you have an Apple Watch paired with your iPhone, you will be able to quickly view messages, upcoming appointments, medications, and alerts right from your watch. For your added convenience, some third-party apps may be compatible for One Chart l Patient.

Can I schedule visits with my provider?

You can request primary care visits directly in One Chart l Patient. Select the time and day you want to see your primary care provider, including your preferred location. Available appointment times are displayed with an up-to-date view of provider schedules. All primary care physicians who you have previously seen are displayed but it is recommended that you schedule with your personal primary care provider who is most familiar with your care.

Will I be able to view the health record of another adult, or family member, in One Chart l Patient?

Yes, you can request proxy access to view the One Chart l Patient account of another individual. The options are to submit electronically through your One Chart l Patient account or to download the printable form from our web site, onechartpatient.com. Some forms may require extra documentation of your relationship to the patient. You will be contacted once the form is reviewed.

How do I communicate my medical concerns?

One Chart l Patient offers a messaging service with your health care team. You are able to communicate with providers that you have had an appointment with in the past three years or upcoming 6 months. We ask that you do not use One Chart l Patient to communicate serious medical concerns with your health care team. Please schedule an appointment with your provider to discuss immediate medical concerns or problems. If you are experiencing a medical emergency, call 911 immediately.
